Approximately 30 million Americans enjoy the benefits of soft contact lenses, and many of them use one or more various lens solutions on a regular basis. They depend on these products and on the companies who make them to help protect their vision. Since 2004, some 2.3 million soft contact lens users in this country have placed their reliance and trust in “ReNu with MoistureLoc;” a topical contact lens solution manufactured by Bausch & Lomb. For many of these ReNu users, this trust may have been tragically misplaced as the product has been linked to a recent worldwide outbreak of Fusarium keratitis. According to the Centers for Disease Control & Prevention (CDC), more than 30% of the Fusarium keratitis cases identified in the United States since the onset of ReNu-generated infections have required corneal transplantation surgery. Some experts have predicted that this figure will increase, and may ultimately reach 50%.
Commonly encountered in plant materials, especially in tropical areas, Fusarium can also be found in drains, including the drains in lavatories where people commonly clean their soft contact lenses. Early diagnosis and treatment of this disease is crucial in avoiding serious vision loss, but diagnosis is often difficult. Fusarium keratitis symptoms include a sudden onset of blurred vision, pain or redness that continues even after the lens is removed, hypersensitivity to light, unusual discharge or tearing and/or lesions on the iris. Unfortunately, antibiotic therapy is useless against pathogenic fungi like Fusarium, and worse, it delays accurate diagnosis and proper treatment.
In November 2005, public health authorities in Hong Kong and in Singapore notified Bausch & Lomb about a sudden and inexplicable increase they had encountered in the number of patients hospitalized with Fusarium keratitis. The only common factor that seemed to link those cases was that most of the patients wore soft contact lens. Although legally required to give notice of these reports within 30 days to the Food and Drug Administration (FDA), Bausch & Lomb failed to do so. This failure to give timely notice was among some 20 potential Bausch & Lomb violations cited by the FDA after inspecting the company’s factory at Greenville, South Carolina. The inspection also resulted in a Bausch & Lomb citation for failure to follow procedures to prevent contamination of equipment at the plant.
In early March 2006, Bausch & Lomb was notified by a New Jersey cornea specialist that three of his recent patients were infected with Fusarium. The company immediately confirmed that all three patients had used ReNu, but apart from this, Bausch & Lomb did not respond to this report. There were no public warnings or announcements, and again, no reports were made to the FDA or to the CDC.
Finally, In April 2006, in the face of reports produced by the FDA and the CDC citing growing evidence of the link between Fusarium keratitis and the use of its product, the company announced suspension of shipments of ReNu with MoistureLoc. In May 2006, Bausch & Lomb made its first public acknowledgment of Fusarium keratitis case reports by ReNu users in Europe, and soon thereafter, the company announced a permanent worldwide withdrawal of ReNu with MoistureLoc from the market. The announcement included the recommendation that consumers stop using the product immediately.
